A Tumble GP has welcomed plans by Hywel Dda University Health Board to develop ten new community hubs to support proposed changes to healthcare services in west Wales.

With less than two weeks to go before Hywel Dda’s Big NHS Change consultation closes, Dr Sioned Richards said the hubs would have the potential to considerably reduce waiting times for patients who have been referred by their GP for a hospital appointment.

Dr Richards said: “I feel the case for change is overwhelming. General Practice is creaking at the seams; we’re not managing demand, appointment times are ridiculously long for patients – we’re simply overwhelmed.

“In terms of waiting times for hospital appointments, we all know what they’re like. We get patients coming in day in, day out, waiting to be seen, for months and months.

“Community hubs will have the potential to really change that. By bringing services back into the communities so that patients get to be seen much quicker.”

Under Hywel Dda’s proposals the ten community hubs would provide a range of health and care services particularly for older, frail and vulnerable people.

Although the care that patients could expect to receive would differ in each individual hub, it could include:

  • Care and support from a range of health and care professionals
  • Local access to tests and scans, including x-rays
  • Outpatients appointments and clinics
  • Care before and after your operations
  • Treatment for a minor injury or illness
  • Planned and preventative care for people living with long term conditions
  • Rehabilitation after a stay in hospital (step-down)
  • An overnight stay in a bed if you can’t be cared for at home but don’t need to go into hospital (step-up)
  • An assisted living bed where you can be supported to live in the community
  • Advice and support around your mental health
  • Access to advice and support on a range of health needs, including information on how to avoid getting ill in the first place, as well as how to get better if you to become ill

Hywel Dda’s Executive Medical Director and Director of Clinical Strategy, Dr Phil Kloer, added: “The whole community model will involve a multi-agency approach including more seamless working between health and social care, along with other agencies and the 3rd sector working together in each locality.

“The priority will be on improving and maintaining health and wellbeing of the population, giving children the best life chances, and promoting healthy ageing, therefore all agencies focused on prevention and early intervention. We know that people’s health and wellbeing is influenced by issues such as social connection and housing, which means that all agencies and people in the community need to be involved to make a positive difference.

“For patients with chronic conditions or who need end of life care, our workforce would be located around the community hubs where they would either be going out to visit people in their homes or people would be able to access the community hubs for a range of assessments, advice, support and treatments.  Our Community Mental health team will also play a key part. We are working on the detail with partners – it’s going to have to be a team effort. But those are the needs of the future population and we need to provide most of that locally because often the things that matter to people aren’t the sort that you need to go to hospital for.”

Mae Meddyg Teulu yn Y Tymbl wedi croesawu cynlluniau gan Fwrdd Iechyd Prifysgol Hywel Dda i ddatblygu deg hyb cymunedol newydd i gefnogi newidiadau arfaethedig i wasanaethau gofal iechyd yng ngorllewin Cymru.

Gyda llai na phythefnos i fynd cyn i ymgynghoriad Hywel Dda – Trawsnewid ein Gwasanaeth Iechyd gau, dywedodd Dr Sioned Richards y byddai gan yr hybiau y potensial i leihau amserau aros yn sylweddol ar gyfer cleifion a sy’n cael eu cyfeirio gan eu Meddyg Teulu am apwyntiad ysbyty.

Dywedodd Dr Richards: “Credaf bod yn rhaid newid. Mae’r pwysau ar y gwasanaeth Meddygon Teulu yn dangos; dydyn ni ddim yn medru ateb y galw, mae amseroedd aros ar gyfer apwyntiadau yn warthus o hir i gleifion – allwn ni ddim cario ‘mlaen fel hyn.

“Mae pawb yn ymwybodol o hyd amserau aros ar gyfer apwyntiadau ysbyty. Rydyn ni’n gweld cleifion bob dydd sy’n aros i gael eu gweld, am fisoedd a misoedd.

“Bydd gan yr hybiau cymunedol y potensial i drawsnewid hyn. Trwy ddychwelyd gwasanaethau i’r cymunedau fel bod cleifion yn cael eu gweld yn llawer cyflymach.”

Dan gynigion Hywel Dda, byddai’r deg hyb cymunedol yn darparu ystod o wasanaethau iechyd a gofal yn enwedig ar gyfer pobl hŷn, pobl fregus a phobl sy’n agored i niwed.

Er y bydd y gofal y gall cleifion ddisgwyl ei gael yn gwahaniaethu o hyb i hyb, gallai gynnwys:

  • Gofal a chefnogaeth gan ystod o weithwyr proffesiynol iechyd a gofal
  • Profion a sganiau yn lleol, yn cynnwys pelydrau-x
  • Apwyntiadau a chlinigau cleifion allanol
  • Gofal cyn ac ar ôl eich llawdriniaeth
  • Triniaeth ar gyfer mân anaf neu salwch
  • Gofal wedi’i gynllunio a gofal ataliol ar gyfer pobl sy’n byw gyda chyflyrau hir-dymor
  • Adsefydlu ar ôl arhosiad mewn ysbyty (gofal ‘cam-i-lawr’)
  • Arhosiad dros nos mewn gwely os na allwch chi gael gofal yn y cartref ond nid oes angen i chi fynd i ysbyty (gofal ‘cam-i-fyny’)
  • Gwely byw â chymorth lle gallwch chi gael cymorth i fyw yn y gymuned
  • Cyngor a chefnogaeth ar eich iechyd meddal
  • Cyngor a chefnogaeth ar ystod o anghenion iechyd, yn cynnwys gwybodaeth ar sut i osgoi mynd yn sâl yn y lle cyntaf, yn ogystal â sut i wella os byddwch chi’n mynd yn sâl

Ychwanegodd Dr Phil Kloer, Cyfarwyddwr Meddygol Gweithredol a Chyfarwyddwr Strategaeth Glinigol Hywel Dda: “Bydd y model cymuned gyfan yn cynnwys ymdriniaeth aml-asiantaethol yn cynnwys mwy o weithio di-dor rhwng iechyd a gofal cymdeithasol ynghyd ag asiantaethau eraill a sefydliadau trydydd sector yn cydweithio ym mhob ardal.

“Y flaenoriaeth fydd gwella a chynnal iechyd a llesiant y boblogaeth, gan roi’r cyfleoedd bywyd gorau i blant, a hyrwyddo heneiddio’n iach, sy’n golygu gweld pob asiantaeth yn canolbwyntio ar atal ac ymyrraeth gynnar. Gwyddom fod materion megis cysylltiad cymdeithasol a thai yn dylanwadu ar iechyd a llesiant pobl, felly mae angen i bob asiantaeth a phobl yn y gymuned fod yn rhan o wneud gwahaniaeth cadarnhaol.

“Ar gyfer cleifion â chyflyrau cronig neu sydd angen gofal diwedd oes, byddai ein gweithlu wedi’u lleoli o amgylch yr hybiau cymunedol, lle byddent naill ai’n mynd i ymweld â phobl yn eu cartrefi neu byddai pobl yn gallu dod i’r hybiau cymunedol hyn ar gyfer ystod o asesiadau, cyngor, cefnogaeth a thriniaethau. Bydd ein Tîm Iechyd Meddwl Cymunedol hefyd  yn chwarae rhan allweddol yn hyn. Rydym yn gweithio ar y manylion gyda phartneriaid – bydd yn rhaid iddo fod yn ymdrech tîm. Ond dyna anghenion y genhedlaeth nesaf ac mae angen i ni ddarparu rhan helaeth y gwasanaeth yn lleol, gan nad yw’r rhan fwyaf o’r pethau sy’n bwysig i bobl yn bethau y mae angen i chi fynd i’r ysbyty yn eu cylch.”
