Patient Knows Best Service User

Over 2,000 people who receive care from Hywel Dda UHB’s respiratory service will receive a letter in the next few days inviting them to start using Patients Know Best (PKB),  a new online service designed to improve your patient experience and access to NHS services and information.

Patients Know Best is a secure UK based system that allows you to access information in relation to your health anywhere, at any time and puts you, the patient, in charge of your medical data. Your record is encrypted so that only you and those you give permission to can access it.

The system will also provide a secure online space to view your appointments letters; allow you to track and monitor your health and symptoms; and provide access to a library of health resources to aid in managing your health.

Chief Executive of Hywel Dda UHB, Steve Moore, said: “It is fantastic to be able to offer this new online service for our respiratory patients and we hope to roll this out to other services as soon as possible.

“Making better use of technology to enable our patients to access care and information has been a key priority of ours and has only been accelerated by the pandemic.

“I’d like to thank all the teams involved in getting Patients Know Best online in Hywel Dda and strongly encourage our patients to sign up if you receive an invitation from us.”

If you receive an invitation letter from us to register with Patient Knows Best, your letter will contain an invite and access code to join.

Patient feedback on Patient Knows Best has demonstrated: “Really useful and easy to use and a great way to check in when advice needed” and “personally the PKB app is a most useful tool easy to use, and to be able to communicate with professional people is a great help.”

Bydd dros 2,000 o bobl sy’n derbyn gofal gan wasanaeth anadlol Bwrdd Iechyd Prifysgol Hywel Dda yn derbyn llythyr yn ystod yr ychydig ddyddiau nesaf yn eu gwahodd i ddechrau defnyddio Patients Know Best (PKB), gwasanaeth ar-lein newydd sydd wedi’i gynllunio i wella profiad claf a mynediad at wasanaethau a gwybodaeth y GIG.

System ddiogel yw Patients Know Best sy’n eich galluogi i gael gafael ar wybodaeth mewn perthynas â’ch iechyd yn unrhyw le, ar unrhyw adeg ac sy’n eich rhoi chi, y claf, yng ngofal o’ch data meddygol. Mae eich cofnod wedi’i amgryptio fel mai dim ond chi a’r rhai rydych chi’n rhoi caniatâd iddynt sy’n cael mynediad iddo.

Bydd y system hefyd yn darparu lle diogel ar-lein i weld eich llythyrau apwyntiadau; caniatáu ichi olrhain a monitro eich iechyd a’ch symptomau; a darparu mynediad i lyfrgell o adnoddau iechyd i gynorthwyo i reoli eich iechyd.

Dywedodd Prif Weithredwr Bwrdd Iechyd Prifysgol Hywel Dda Steve Moore: “Mae’n wych gallu cynnig y gwasanaeth ar-lein newydd hwn i’n cleifion anadlol ac rydym yn gobeithio ei gyflwyno i wasanaethau eraill cyn gynted â phosibl.

“Mae gwneud gwell defnydd o dechnoleg i alluogi bod ein cleifion yn cael mynediad at ofal a gwybodaeth wedi bod yn flaenoriaeth allweddol i ni ac mae’r pandemig wedi ei gyflymu.

“Hoffwn ddiolch i’r holl dimau sy’n ymwneud â sicrhau bod Patients Know Best  ar-lein yn Hywel Dda ac annogaf  ein cleifion yn gryf i arwyddo os ydych chi’n derbyn gwahoddiad gennym ni.”

Os ydych chi’n derbyn llythyr gwahoddiad gennym ni i gofrestru gyda Patient Knows Best, bydd eich llythyr yn cynnwys cod gwahoddiad a mynediad i ymuno.

Mae adborth cleifion ar Patient Knows Best wedi dangos: “Yn wirioneddol ddefnyddiol ac yn hawdd ei ddefnyddio ac yn ffordd wych o wirio pryd mae angen cyngor” ac “yn bersonol mae’r ap PKB yn adnodd defnyddiol iawn sy’n hawdd ei ddefnyddio, ac i allu cyfathrebu â gweithiwr proffesiynol yn help mawr. ”
