With autumn firmly on our doorstep and winter fast approaching we are totally focused on preparing for the winter challenges ahead together with our communities, staff, volunteers, carers and partners.

The onset of the COVID pandemic has forced us to behave differently, in ways that many of us are still trying to get used to. 2020 has been a year in which we have had to fundamentally change the way we live, work, interact with and care for each other.

Tragically, there continues to be a real human cost to the pandemic and many families have lost loved ones before their time. Many others have survived, but the journey to recovery continues for them – and for us.

Everyone is naturally tired and weary.  The disease is resurgent and we are once again on the cusp of a national “firebreak” lockdown across the whole of Wales to help stem the spread of the virus and allow our health service to look after the increasing number of people who will fall seriously ill. This will have an impact on our staff and communities.  It affects NHS services and social care at a time of the year when services are often under extreme pressure.

Our workforce has worked tirelessly to change the way that we provide our services and care for people. We have worked hard to stay ahead of the curve since the onset of the pandemic.

In the first wave we set up Wales’ first Community Testing Units, and a local command centre to provide critical information and guidance to our staff and partners.

We changed our hospital, primary care and community environments to abide by new regulations to keep people safe. At the same time we have continued to deliver emergency and critical care safely where needed the most.

We’ve sourced and distributed Personal Protection Equipment in the most challenging global market. We delivered field hospitals and have retained much of the expanded our bed base ready for a second spike.

Digital solutions in primary and secondary care have been put in place to transform the way that we interact with our patients on telephone and devices. Our ICT staff have delivered over 220 tablets to help inpatients to keep in touch with family and friends while visiting is so restricted and families are separated.  Our family liaison officers are trying to maintain that essential contact with loved ones.

Our GPs, hospital clinicians, care home commissioners and redeployed staff are continuing to support our care homes residents and staff, including a mammoth testing programme.

We’ve contributed to research and development into treating COVID19 and will continue to do so.

We have greater access to testing now, with enhanced arrangements in Aberystwyth and Llanelli. There is adequate local capacity for testing, despite some issues with the UK portal, and we’re also working on a more hybrid system, to run alongside the UK portal that will enable us to offer more local testing for our communities

We have been building on our Test, Trace, Protect strategy through our TTP Regional Response Cell, and we are working closely with Public Health Wales and all of our local authority partners who lead the local contact tracing teams, helping us to spot clusters early and break chains of transmission. Over 90 per cent of our contacts have been successfully traced and we thank you all for your continued help in working with our teams.

Our focus is also on re-establishing elective care where we can as we live alongside COVID19 and this has started in our hospitals. For many people this has been the main impact on them of the pandemic.  We intend to contact each person individually to explain why there is a delay, what they can do to self- manage their condition and what they can do should they deteriorate and when they can expect their operation.

We know the impact of this will be felt for years to come, but we have been astounded with the support for our NHS in Hywel Dda.

It is more important than ever at this time to get protection against respiratory illness and our GPs and pharmacists and teams are working tirelessly to enable people in the priority groups to receive their flu jabs safely.

In the first wave of the pandemic some people were afraid to access health care. Our infection prevention and control teams, our housekeeping staff and all our workforce are making our hospitals as safe as possible. We are there for you if you need us.

It sometimes harder, as time goes on, to remember to stick to the basics that will keep us safer. These are:

  • Always observe physical distancing (two metres).
  • Wear a mask or face covering when you are in indoor public areas like shops.
  • Wash your hands regularly. Hand sanitiser is great when you are out and about.
  • Stay home and arrange to get a test if you develop any of the following symptoms: a high temperature, a new continuous cough and a loss or change of taste or smell

The Welsh Government has recognised the efforts of Hywel Dda Health Board recently by taking us out of targeted intervention. This is a tribute to all our staff and their tireless dedication and expertise.

Every pandemic in the history of mankind comes to an end. This one will too. We are prepared for the worst but are working for the best possible outcomes for our population. Together we will get through this winter stronger than ever.

Gyda’r hydref ar drothwy’r drws a’r gaeaf yn gyflym agosáu rydym yn canolbwyntio’n llwyr ar baratoi ar gyfer heriau’r gaeaf sydd o’n Blaenau ochr yn ochr â’n cymunedau, staff, gwirfoddolwyr, gofalwyr a phartneriaid.

Mae dyfodiad y pandemig COVID wedi ein gorfodi i ymddwyn yn wahanol, mewn ffyrdd y mae llawer ohonom yn dal i geisio dod i arfer â nhw. Mae 2020 wedi bod yn flwyddyn y bu’n rhaid i ni newid yn sylfaenol ein ffordd o fyw, gweithio, rhyngweithio a gofalu am eim gilydd.

Yn drasig iawn, mae cost ddynol wirioneddol y pandemig yn parhau ac mae llawer o deuluoedd wedi colli anwyliaid cyn eu hamser. Mae llawer o rai eraill wedi goroesi, ond mae’r daith o adfer yn parhau iddynt – ac i ni.

Mae pawb yn naturiol wedi blino. Mae’r afiechyd yn adfywio ac rydym unwaith eto ar drothwy clo cenedlaethol ledled Cymru i helpu i atal y firws rhag lledaenu ac i ganiatáu i’n gwasanaeth iechyd ofalu am y nifer cynyddol o bobl a fydd yn mynd yn ddifrifol wael. Bydd hyn yn cael effaith ar ein staff a’n cymunedau. Mae’n effeithio ar wasanaethau’r GIG a gofal cymdeithasol ar adeg o’r flwyddyn pan mae gwasanaethau yn aml dan bwysau eithafol.

Mae ein gweithlu wedi gweithio’n ddiflino i newid y ffordd yr ydym yn darparu ein gwasanaethau ac yn gofalu am bobl. Rydym wedi gweithio’n galed i weithredu’n rhagweithiol ers dyfodiad y pandemig.

Yn y don gyntaf fe wnaethom sefydlu Unedau Profi Cymunedol cyntaf Cymru, a chanolfan reoli leol i ddarparu gwybodaeth ac arweiniad beirniadol i’n staff a’n partneriaid.

Fe wnaethom newid ein hamgylcheddau ysbyty, gofal sylfaenol a chymunedol i gadw at reoliadau newydd er mwyn cadw pobl yn ddiogel. Ar yr un pryd, rydym wedi parhau i ddarparu gofal brys a gofal critigol mewn ffyrdd diogel lle bo’r angen fwyaf.

Rydym wedi dod i ben â chael a dosbarthu Offer Amddiffyn Personol mewn marchnad fyd-eang heriol iawn. Fe wnaethom gyflenwi ysbytai maes ac rydym wedi cadw llawer o’r sylfaen fwy o welyau mewn paratoad ar gyfer ail don.

Mae datrysiadau digidol mewn gofal sylfaenol ac eilaidd wedi bod ar waith i drawsnewid y ffordd yr ydym yn rhyngweithio â’n cleifion ar y ffôn ac ar ddyfeisiau. Mae ein staff TGCh wedi dosbarthu dros 220 o lechi i helpu cleifion mewnol i gadw mewn cysylltiad â theulu a ffrindiau tra bod cyfyngiadau ar ymweld. Mae ein swyddogion cyswllt teulu yn ceisio cynnal y cyswllt hanfodol hwnnw ag anwyliaid.

Mae ein meddygon teulu, clinigwyr ysbytai, comisiynwyr cartrefi gofal a staff sydd wedi’u hadleoli yn parhau i gefnogi preswylwyr a staff ein cartrefi gofal, gan gynnwys rhaglen brofi enfawr.

Rydym wedi cyfrannu at ymchwil a datblygu i drin COVID19 a byddwn yn parhau i wneud hynny.

Bellach, mae gennym fwy o fynediad at brofion, gyda threfniadau gwell yn Aberystwyth a Llanelli. Mae capasiti lleol digonol i brofi, er gwaethaf rhai problemau gyda phorth y DU, ac rydym hefyd yn gweithio ar system fwy hybrid, i redeg ochr yn ochr â phorth y DU a fydd yn ein galluogi i gynnig mwy o brofion lleol i’n cymunedau.

Rydym wedi bod yn adeiladu ar ein strategaeth profi, olrhain, amddiffyn, trwy ein Cell Ymateb Rhanbarthol, ac rydym yn gweithio’n agos gydag Iechyd Cyhoeddus Cymru a phob un o’n partneriaid awdurdod lleol sy’n arwain y timau olrhain cyswllt lleol, gan ein helpu i ddod o hyd i glystyrau yn gynnar, a thorri cadwyni trosglwyddo. Mae dros 90% o’n cysylltiadau wedi cael eu holrhain yn llwyddiannus ac rydym yn diolch i chi i gyd am eich help parhaus wrth weithio gyda’n timau.

Rydym hefyd yn canolbwyntio ar ailsefydlu gofal dewisol lle y gallwn wrth i ni fyw ochr yn ochr â COVID19 ac mae hyn wedi cychwyn yn ein hysbytai. I lawer o bobl dyma fu’r prif effaith y pandemig arnynt. Rydym yn bwriadu cysylltu â phob unigolyn yn unigol i egluro pam bod oedi, beth allan nhw ei wneud i hunanreoli eu cyflwr, beth allan nhw ei wneud petai nhw’n dirywio a phryd y gallan nhw ddisgwyl eu llawdriniaeth.

Rydym yn gwybod y bydd effaith hyn yn cael ei deimlo am flynyddoedd i ddod, ond rydym wedi ein syfrdanu gyda’r gefnogaeth i’n Gwasanaeth Iechyd yn Hywel Dda.

Ar hyn o bryd, mae’n bwysicach nag erioed i amddiffyn rhag salwch anadlol ac mae ein meddygon teulu, ein fferyllwyr a’n timau’n gweithio’n ddiflino i alluogi pobl yn y grwpiau blaenoriaeth i gael y brechiad ffliw yn ddiogel.

Yn nhon gyntaf y pandemig roedd rhai pobl yn ofni defnyddio gwasanaethau gofal iechyd. Mae ein timau atal a rheoli heintiau, ein staff gofal tŷ a’n holl weithlu yn gwneud ein hysbytai mor ddiogel â phosib. Rydym yma i chi os ydych chi ein hangen ni.

Weithiau mae’n anoddach, wrth i amser fynd yn ei flaen, i gofio cadw at y pethau sylfaenol a fydd yn ein cadw’n fwy diogel. Sef:

• Cadw pellter corfforol bob amser (dau fetr).
• Gwisgo mwgwd neu orchudd wyneb pan fyddwch chi mewn mannau cyhoeddus dan do fel siopau.
• Golchi dwylo yn rheolaidd. Mae glanweithydd dwylo yn wych pan rydych chi allan.
• Aros gartref a threfnu i gael prawf os byddwch yn datblygu unrhyw un o’r symptomau canlynol: tymheredd uchel, peswch parhaus newydd a cholli neu newid blas neu arogl

Mae Llywodraeth Cymru wedi cydnabod ymdrechion Bwrdd Iechyd Hywel Dda yn ddiweddar trwy ein tynnu allan o ymyrraeth wedi’i thargedu. Mae hon yn deyrnged i’n holl staff a’u hymroddiad diflino a’u harbenigedd.

Daeth pob pandemig yn hanes y ddynoliaeth i ben. Bydd yr un hon yn dod i ben hefyd. Rydym yn barod am y gwaethaf ond yn gweithio i gael y canlyniadau gorau posib i’n poblogaeth. Gyda’n gilydd byddwn yn dod trwy’r gaeaf hwn yn gryfach nag erioed.
