A new wrap-around support service to help you and your loved ones live independently for longer is now available in Pembrokeshire.
The Delta CONNECT service giving families peace of mind combines an enhanced lifeline and telecare service and aims to help identify any potential health and wellbeing issues at the earliest opportunity.
The service, which is free to join until March 31, 2021, includes pro-active wellbeing calls and Technology Enabled Care (TEC) equipment such as red button lifelines, fall detectors, GPS tracking devices, door sensors, medication dispensers and many more useful devices to assist clients to live the life they want.
As part of the service, clients will also receive digital support, help to re-engage with the local community (virtual currently, but physically longer term) and access to an appropriate community response service if in crisis. Further details about this will be available shortly.
This is the first service of its kind in Wales and it has been funded by the Welsh Government’s Transformation Fund, enabling the West Wales Care Partnership to work together to help shape the future of health and social care services across west Wales.
Speaking about the vital service, Pembrokeshire County Council Director of Social Services Jonathan Griffiths said, “We have a strong, vibrant and active community within Pembrokeshire and we are working closely with partners across health, social care and the third sector to develop a wide range of initiatives aimed at supporting people in their own homes.
“CONNECT is a part of our community offer that allows us to provide the right help, at the right time when people need it. Working together means we aim to find the best solution to support the person and their family. We seek to support people to build their confidence, maintain their independence, and for the person and their families to feel confident they are being supported and are connected.”
