An Executive Summary has been published of an independently authored Serious Case Review into the death of a 77 year-old person in May 2012.
The Serious Case Review (SCR) was carried out under the Wales Interim Policy & Procedures for the Protection of Vulnerable Adults from Abuse (2013).
The primary purpose of a SCR carried out under this guidance is to:
- Establish whether there are lessons to be learned from the case about the way in which local professionals and agencies work together to safeguard vulnerable adults;
- Identify clearly what those lessons are, how they will be acted upon, and what is expected to change as a result;
- Improve inter-agency working, and better protection and support of vulnerable adults.
The report was commissioned by the Pembrokeshire Adult Protection Committee, which accepts the recommendations for multi-agency learning made by the report.
