Adroddiad Blynyddol y Gwasanaethau Cymdeithasol
Mae Gwasanaethau Cymdeithasol Sir Gaerfyrddin wedi derbyn adroddiad blynyddol cadarnhaol am ei berfformiad yn 2016-2017.
Mae’r adroddiad yn dangos bod llai o gleifion yn cael eu cadw i mewn yn yr ysbyty o ganlyniad i oedi wrth drefnu gofal a bod llai o blant yn gorfod cael eu derbyn i ofal.
Mae’r adroddiad gan Jake Morgan, Cyfarwyddwr Gwasanaethau Cymdeithasol y Cyngor Sir, yn rhinwedd ei swyddogaeth fel Cyfarwyddwr Statudol y Gwasanaethau Cymdeithasol, wedi cael ei gyflwyno yng nghyfarfod y cyngor llawn y mis hwn.
Croesawodd werthusiad gan Arolygiaeth Gofal a Gwasanaethau Cymdeithasol Cymru, a oedd yn datgan: “Mae’r Awdurdod Lleol yn parhau i elwa ar arweinyddiaeth gref ar draws y gwasanaethau oedolion a phlant, gyda sefydlogrwydd da a chymorth effeithiol ar gyfer ei weithlu.
”Cynnydd da o ran gweithredu Deddf Gwasanaethau Cymdeithasol a Llesiant (Cymru), ac mae hyn wedi rhoi cryn bwysigrwydd ar sicrhau bod ffyrdd newydd o weithio yn cael eu deall gan staff ac asiantaethau partner a’u rhoi ar waith a buddsoddiad i wireddu hynny. Mae bwrdd prosiect ar waith gyda chynllun gweithredu clir sy’n cyd-fynd â’r Ddeddf Gwasanaethau Cymdeithasol a Llesiant, ac mae cerrig milltir pwysig wedi ac yn parhau i gael eu cyflawni.”
Mae adroddiad y cyfarwyddwr yn datgelu bod gwelliannau’r gwasanaethau atal i blant wedi bod yn llwyddiant go iawn wrth i fwy o blant gael eu cadw gartref.
Mae fframwaith comisiynu ar gyfer gofal cartref wedi cael ei sefydlu sy’n caniatáu mwy o hyblygrwydd er mwyn sicrhau nad yw ymweliadau â phobl fregus yn cael eu cwtogi.
Dywedodd Mr Morgan: “Byddwn hefyd yn cynyddu’r cyflenwad o ofal ychwanegol drwy’r datblygiad Llynnoedd Delta, gan adeiladu ar lwyddiant y datblygiadau gofal ychwanegol yn Rhydaman a Chaerfyrddin. Rydym yn cadw ein darpariaeth breswyl fewnol bresennol wrth ystyried pa fuddsoddiad cyfalaf fydd ei angen i wella’r amgylcheddau ffisegol.”
Gwelwyd gostyngiad yn nifer yr achosion o oedi cyn trosglwyddo gofal ar gyfer cleifion preswyl mewn ysbytai am resymau sy’n ymwneud â gofal cymdeithasol megis aros am addasiadau i’r cartref, lleoliadau gofal preswyl neu gyllid. Mae’r adroddiad hefyd yn nodi meysydd i’w gwella, gan gynnwys yr angen i gynnal adolygiadau pecynnau gofal yn fwy aml.
Dywedodd y Cynghorydd Jane Tremlett, yr Aelod o’r Bwrdd Gweithredol dros Ofal Cymdeithasol ac Iechyd: “Mae hwn yn adroddiad cadarnhaol a gaiff ei ddylanwadu gan gyd-bwyllgor craffu ardderchog ac arweinyddiaeth gref ar draws gwasanaethau oedolion a phlant. Diolchaf i’n holl staff gofal cymdeithasol am eu gwasanaethau priodol ac amserol ar gyfer pobl Sir Gaerfyrddin. Mae llesiant yr unigolyn wrth wraidd popeth a wnawn, gan sicrhau bod ein cymunedau yn gymunedau gwydn.”

Social Services Annual Report
Carmarthenshire Social Services have been given a positive annual report for their performance in 2016-17.
It shows that fewer hospital patients are being kept in because of delays in arranging care and fewer children are having to be taken into care.
The report by the county council’s director of community services Jake Morgan, in his capacity of statutory director of social services, has been presented to this month’s meeting of the full council.
He welcomed an evaluation by the Care and Social Services Inspectorate Wales which said: “The local authority continues to benefit from strong leadership across adults and children’s services, with good stability and effective support for its workforce.
”Good progress on the implementation of the Social Services and Well-Being (Wales) Act (SSWBA), and has placed significant importance and investment on ensuring new ways of working are understood by staff and partner agencies and embedded into practice. A project board is in place with a clear action plan aligned to the SSWBA, and key milestones have been and continue to be achieved.”
The director’s report reveals that the improvement of preventative services for children has been a real success with more children being kept at home.
A commissioning framework for domiciliary care allows more flexibility so that visits to vulnerable people are not cut short.
Mr Morgan said: “We will further increase the supply of extra care through the Delta Lakes development, building on the success of the Extra care developments in Ammanford and Carmarthen. We are retaining our current in house residential provision whilst considering what capital investment will be needed to improve the physical environments.”
There has been a reduction in the number of delayed transfers of care for hospital inpatients for social care reasons such as waiting for home adaptations, residential care placements, or funding. The report also identifies areas for improvement including the need to improve the regularity of reviews of care packages.
The executive board member for social care and health Cllr Jane Tremlett said: “This is a positive report influenced by an excellent joint scrutiny committee and strong leadership across adult and children’s services. I thank all our social services staff for their appropriate and timely services to the people of Carmarthenshire. At the heart of all we do is the wellbeing of the individual, ensuring our communities are resilient communities.”
