Pioneering community scheme to support more people to lead a good life
Neath Port Talbot Council has appointed two additional Local Area Coordinators to expand its pioneering community support scheme.
The two additional Coordinators will cover the areas of Onllwyn, Seven Sisters, Crynant, Glynneath and Banwen, and the areas of Bryn, Cwmavon and Port Talbot town.
Local Area Coordinators help people to avoid reaching a crisis in their life or help people to recover if a crisis has already happened. The Coordinators work with people who need support due to their age, disability or mental health to build a vision for a good life.
Using local knowledge, the Coordinators work with people and their families to identify and access resources to help them stay strong. This could include working with groups, services and organisations such as local GPs, councillors, volunteer groups, housing officers, third sector organisations and businesses. The Coordinators also work closely with the Community Resource Team, a joint service between the Council and ABMU which helps people who need support to stay independent in their own homes.
Councillor Peter Richards, Cabinet Member for Adult Social Care and Health, said:
“Local Area Coordination is one of our key priority programmes to help prevent, delay and reduce the demand for people needing costly social care and health services.
“Our Coordinators can help people to help themselves through using their wealth of local knowledge and connecting people to existing resources in the community. We want to work alongside people to increase their independence and personal resilience so they can lead the best life possible.”
The support from a Local Area Coordinator is free; there are no assessments or referral process, and no time limits.

Mae Cyngor Castell-nedd Port Talbot wedi penodi dau Gydlynydd Ardal Leol ychwanegol i ehangu ei gynllun cefnogaeth gymunedol arloesol.
Bydd y ddau gydlynydd ychwanegol yn gyfrifol am ardaloedd Onllwyn, Blaendulais, y Creunant a Banwen, yn ogystal â Bryn, Cwmafan a thref Port Talbot.
Mae Cydlynwyr ArdaloeddLleol yn helpu pobl i osgoi argyfwng yn eu bywydau neu’n eu helpu i ymdopi os oes argyfwng wedi digwydd yn barod. Mae’r cydlynwyr yn gweithio gyda phobl y mae angen cefnogaeth arnynt oherwydd oedran, anabledd neu iechyd meddwl i lunio cynllun am fywyd da.
Gan ddefnyddio gwybodaeth leol, mae’r cydlynwyr yn gweithio gyda phobl a’u teuluoedd i ganfod a chael mynediad i adnoddau i’w helpu nhw i barhau i fod yn gryf. Gallai hyn gynnwys gweithio gyda grwpiau, gwasanaethau a sefydliadau megis meddygon teulu lleol, cynghorwyr, grwpiau gwirfoddol, swyddogion tai, sefydliadau trydydd sector a busnesau. Mae’r cydlynwyr hefyd yn gweithio’n agos gyda’r Tîm Adnoddau Cymunedol; dyma wasanaeth ar y cyd rhwng y cyngor a PABM sy’n helpu pobl y mae angen cefnogaetharnynt er mwyn aros yn annibynnol yn eu cartrefi eu hunain.
Meddai’r Cynghorydd Peter Richards, Aelod y Cabinet dros Ofal Cymdeithasol i Oedolion ac Iechyd,
“Mae Cydlynu ArdaloeddLleol yn un o’n rhaglenni allweddol i helpu i atal, gohirio a lleihau’r galw am ofal cymdeithasol costus a gwasanaethau iechyd.”
“Gall ein cydlynwyr helpu pobl i’w helpu eu hunain trwy ddefnyddio eu gwybodaeth leol eang i gysylltu pobl ag adnoddau sydd eisoes ar gael yn y gymuned. Rydyn ni am weithio gyda phobl i gynyddu eu hannibyniaeth a’u gwytnwch personol fel eu bod yn gallu byw’r bywyd gorau posib.”
