Carmarthenshire heart failure service shortlisted in British Heart Foundation’s prestigious awards

CARMARTHENSHIRE HEART FAILURE SERVICE SHORTLISTED IN BRITISH HEART FOUNDATION’S PRESTIGIOUS AWARDS

 

A team providing care and support to patients with heart failure, and their carers has been shortlisted for the British Heart Foundation (BHF) Alliance Awards at the annual British Cardiovascular Society (BCS) conference.

 

The BHF Alliance Awards is an annual ceremony that celebrates the inspirational work of healthcare professionals from across the UK in improving care for patients suffering from or at risk of heart and circulatory disease.

 

The Carmarthenshire heart failure service is nominated in the Team of the Year category for their work in supporting the needs of patients and their carers from diagnosis through palliative care and death. They will travel to the BHF Alliance awards in Manchester for the final ceremony on Tuesday 5 June.

 

The awards are judged in two separate stages by a panel of cardiology and healthcare experts who rate nominations on several categories, including how teams and individuals have gone above and beyond the basic requirements of their role.

 

The awards, presented by Simon Gillespie, Chief Executive of the BHF which is a key supporter of the conference, recognise teams from across the UK for demonstrating bravery, compassion and drive in their day to day work.

 

The service provided by the Carmarthenshire heart failure team transcends primary, secondary & tertiary care, social care and voluntary agencies facilitating the concept of seamless and integrated management, so that care is provided at “the right time, right place and by the right person”.

 

The team aims to provide a high quality, evidence based service, encapsulating the early stages of diagnosis; optimisation of evidence based pharmacological and self-care strategies and interventional treatments.
