Health board apology after new-born dies in Glangwili Hospital

 

Hywel Dda University Health Board has given an apology about the care and treatment given to a mother and her late son at Glangwili Hospital, Carmarthen.

A complaint by the parents to the health board was upheld by the Public Services Ombudsman for Wales, who pointed to a “host of failings” including that their son’s death had been incorrectly registered as a stillbirth rather than a neonatal death.

The Ombudsman has recommended the sum of £4500 be paid by the health board in recognition of distress, delay and uncertainty experienced in the case. Lessons should be learned within the department according to the Ombudsman.

Mid and West AM Simon Thomas said: “I would like to extend my sympathies to the family involved in these distressing circumstances. I support the Ombudsman’s conclusions and expect the health board to implement the recommendations of the report. I’ll be asking Hywel Dda University Health Board how they intend to respond to the report and hope to raise the issue with the Welsh Government in the Senedd as soon as possible.”

Ymddiheurodd Bwrdd Iechyd Prifysgol Hywel Dda am y gofal a thriniaeth roddwyd i fam a’i diweddar fab yn Ysbyty Glangwili, Caerfyrddin.

 

Mae Ombwdsmon Gwasanaethau Cyhoeddus Cymru wedi cynnal cwyn gan y rhieni, a chyfeiriodd yr Ombwdsmon at “gyfres o fethiannau” gan y bwrdd iechyd gan gynnwys bod marwolaeth eu mab wedi ei gofnodi yn anghywir fel marw-anedig yn hytrach na marwolaeth newydd-anedig.

 

Awgrymodd yr Ombwdsmon y dylai’r swm o £4500 gael ei dalu gan y Bwrdd Iechyd i gydnabod y trallod, yr oedi a’r amwysedd a brofwyd yn yr achos hwn. Yn ôl yr Ombwdsmon, dylai gwersi gael eu dysgu o fewn yr adran.

 

Dywedodd Simon Thomas, AC y Canolbarth a’r Gorllewin: “Hoffwn gydymdeimlo’n daer â’r teulu sydd ynghlwm â’r sefyllfa drallodus hon. Rwy’n cefnogi canlyniadau’r Ombwdsmon ac yn disgwyl i’r bwrdd iechyd weithredu ar awgrymiadau’r adroddiad. Byddaf yn gofyn i Fwrdd Iechyd Prifysgol Hywel Dda sut y maent yn bwriadu ymateb i’r adroddiad ac yn gobeithio codi’r mater gyda Llywodraeth Cymru yn y Senedd cyn gynted â phosibl.”
