Community nurses play an essential role in providing quality and timely care to patients every day across Carmarthenshire, Ceredigion and Pembrokeshire, and even more so now as we respond to the current Coronavirus pandemic.
An indispensable service to all our communities in the Hywel Dda area, our community nurses provide professional advice and a comprehensive range of treatments/interventions to support local patients. This includes helping people receive care closer to home, avoiding the need to go into hospital when it’s not necessary; supporting people to manage their chronic conditions; and helping to safely transfer people back into the community after a hospital stay. They also enthusiastically embrace opportunities to improve our patients’ experience and work hard every day to deliver the highest quality, safest care possible for our local population.
Mandy Rayani, Director of Nursing, Quality & Patient Experience at Hywel Dda University Health Board said: “I’m incredibly proud of all our nursing staff here in Hywel Dda and I’m pleased that we’ve seen a lot of support for our NHS staff from the public, current and former staff and partners, as it really helps staff morale during this challenging time.
“Unfortunately, we have also received reports recently of community staff being challenged about wearing their uniform outside hospitals or other healthcare settings when undertaking their caring role and consequently they are left feeling vulnerable. I am sure that some of this challenge is due to people’s anxiety and concern for their own health and well-being, however, our community health workers are required to travel between patients in their uniform.
“To protect themselves, their patients and the public, all our community nurses provide care and treatment in line with national infection prevention and control practices and guidelines. Whilst it is best practice for hospital-based staff to change into and out of uniforms and not wear them when travelling to and from work, this does not apply to community health workers. The evidence of an infection risk associated with the wearing of the uniform when staff follow personal protective equipment (PPE) is low.”
Hywel Dda University Health Board continues to regularly review all PPE and other clinical guidance to ensure our population are supported in the most effective and evidence based way.

- wash your hands with soap and water often – do this for at least 20 seconds
- always wash your hands when you get home or into work
- use hand sanitiser gel if soap and water are not available
- cover your mouth and nose with a tissue or your sleeve (not your hands) when you cough or sneeze
- put used tissues in the bin straight away and wash your hands afterwards
- try to avoid close contact with people who are unwell
- do not touch your eyes, nose or mouth if your hands are not clean

Mae nyrsys cymunedol yn chwarae rhan hanfodol wrth ddarparu gofal amserol o ansawdd i gleifion bob dydd ar draws Sir Gaerfyrddin, Ceredigion a Sir Benfro, a hyd yn oed yn fwy felly nawr wrth i ni ymateb i’r pandemig Coronafirws cyfredol.
Yn wasanaeth hanfodol i’n holl gymunedau yn ardal Hywel Dda, mae ein nyrsys cymunedol yn darparu cyngor proffesiynol ac ystod gynhwysfawr o driniaethau / ymyriadau i gefnogi cleifion lleol. Mae hyn yn cynnwys helpu pobl i dderbyn gofal yn nes at adref, gan osgoi’r angen i fynd i’r ysbyty pan nad yw’n angenrheidiol; cefnogi pobl i reoli eu cyflyrau cronig; a helpu i drosglwyddo pobl yn ôl i’r gymuned yn ddiogel ar ôl aros yn yr ysbyty. Maent hefyd yn croesawu cyfleoedd yn frwd i wella profiad ein cleifion ac yn gweithio’n galed bob dydd i ddarparu’r gofal mwyaf diogel o’r ansawdd uchaf posibl i’n poblogaeth leol.
Dywedodd Mandy Rayani, Cyfarwyddwr Nyrsio, Ansawdd a Phrofiad Cleifion ym Mwrdd Iechyd Prifysgol Hywel Dda: “Rwy’n hynod falch o’n holl staff nyrsio yma yn Hywel Dda ac rwy’n falch ein bod wedi gweld llawer o gefnogaeth i’n Staff y GIG gan y cyhoedd, staff presennol a blaenorol a phartneriaid, gan ei fod yn helpu morâl staff yn ystod yr amser heriol hwn.
“Yn anffodus, rydym hefyd wedi derbyn adroddiadau yn ddiweddar am staff cymunedol yn cael eu herio ynglŷn â gwisgo eu gwisg y tu allan i ysbytai neu leoliadau gofal iechyd eraill wrth ymgymryd â’u rôl ofalu ac o ganlyniad maent yn cael eu gadael yn teimlo’n fregus. Rwy’n siŵr bod peth o’r her hon oherwydd pryder a phryder pobl am eu hiechyd a’u llesiant eu hunain, fodd bynnag, mae’n ofynnol i’n gweithwyr iechyd cymunedol deithio rhwng cleifion yn eu gwisg.
“Er mwyn amddiffyn eu hunain, eu cleifion a’r cyhoedd, mae ein holl nyrsys cymunedol yn darparu gofal a thriniaeth yn unol ag arferion a chanllawiau cenedlaethol ar gyfer atal a rheoli heintiau. Er ei bod yn arfer gorau i staff mewn ysbytai newid i mewn ac allan o wisgoedd a pheidio â’u gwisgo wrth deithio i’r gwaith ac yn ôl, nid yw hyn yn berthnasol i weithwyr iechyd cymunedol. Mae’r dystiolaeth o risg haint sy’n gysylltiedig â gwisgo’r wisg pan fydd staff yn gwisgo offer amddiffyn personol (PPE) yn isel. ”
Mae Bwrdd Iechyd Prifysgol Hywel Dda yn parhau i adolygu’r holl ganllawiau PPE a chanllawiau clinigol eraill yn rheolaidd i sicrhau bod ein poblogaeth yn cael eu cefnogi yn y ffordd fwyaf effeithiol sy’n seiliedig ar dystiolaeth.
