New online service introduced to monitor heart health between appointments | Gwasanaeth ar-lein newydd wedi’i gyflwyno i fonitro iechyd y galon rhwng apwyntiadau

Hywel Dda University Health Board has introduced Digital Assessments; a new way to help patients keep us informed about their health in-between appointments.

Suitable patients receiving care from our Heart Function Service will receive a text messages or email with a nhs.my link to a Digital Assessment form to answer questions about symptoms, physical health and quality of life.

The team responsible for your care will be able to review the information provided in the Digital Assessment, allowing them to check on your progress and understand your health in-between your appointments.

The nhs.my link in the text message and email is trustworthy and delivered by DrDoctor, a service accredited to the highest standards set by the NHS for protecting the healthcare information of UK citizens.

The information provided will be stored securely within your patient record.

Dr Phil Kloer, Deputy Chief Executive and Medical Director at Hywel Dda UHB, said: “It is fantastic that we are able to introduce Digital Assessments for the first time in Hywel Dda UHB for suitable patients under the care of our Heart Function Service.

“Digital Assessments do not replace medical care. The information provided through this secure platform will provide regular monitoring of symptoms at home to help us better understand your health and make the best use of the time spent with your clinical team.

“We hope to expand this service soon to different clinics and services including trauma and orthopaedics and eye care services.”

If your doctor or nurse has identified you as a suitable patient to use Digital Assessments you will automatically receive a text or email asking you to complete the online form in-between your appointments.

It is important that we have your correct mobile number, full name, postcode, date of birth and email so you can log in to complete your online form. If you need to update your contact details please phone us on 0300 303 9642.

Bydd cleifion addas sy’n derbyn gofal gan ein Gwasanaeth Swyddogaeth y Galon yn derbyn negeseuon testun neu e-bost gyda dolen nhs.my i ffurflen Asesu Digidol i ateb cwestiynau am symptomau, iechyd corfforol ac ansawdd bywyd.

Bydd y tîm sy’n gyfrifol am eich gofal yn gallu adolygu’r wybodaeth a ddarperir yn yr Asesiad Digidol, gan ganiatáu iddynt wirio’ch cynnydd a deall eich iechyd rhwng eich apwyntiadau.

Mae’r ddolen nhs.my yn y neges destun a’r e-bost yn ddibynadwy ac yn cael ei ddarparu gan DrDoctor, gwasanaeth sydd wedi’i achredu i’r safonau uchaf a osodwyd gan y GIG ar gyfer amddiffyn gwybodaeth gofal iechyd dinasyddion y DU.

Bydd y wybodaeth a ddarperir yn cael ei storio’n ddiogel y eich cofnod claf.

Dywedodd Dr Phil Kloer, Dirprwy Brif Weithredwr a Chyfarwyddwr Meddygol Bwrdd Iechyd Prifysgol Hywel Dda: “Mae’n wych ein bod yn gallu cyflwyno Asesiadau Digidol am y tro cyntaf ym Mwrdd Iechyd Prifysgol Hywel Dda ar gyfer cleifion addas sydd o dan ofal ein Gwasanaeth Swyddogaeth y Galon.

“Nid yw Asesiadau Digidol yn disodli gofal meddygol. Bydd y wybodaeth a ddarperir trwy’r platfform diogel hwn yn monitro symptomau gartref yn rheolaidd i’n helpu i ddeall eich iechyd yn well a gwneud y defnydd gorau o’r amser a dreulir gyda’ch tîm clinigol.

“Rydyn ni’n gobeithio ehangu’r gwasanaeth hwn yn fuan i wahanol glinigau a gwasanaethau gan gynnwys gwasanaethau trawma ac orthopaedeg a gofal llygaid.”
