Hywel Dda UHB reminds patients to bring their medication to hospitalĀ

When coming to hospital it is important for patients to bring their medication with them, both prescribed and bought over the counter.

Hywel Dda University Health Board (UHB) is encouraging the public to bring in all medications when attending hospital including inpatient stays, outpatient clinic appointments, pre-op assessments and out-of-hours medical service to help ensure patient safety.

An accurate list of your medicines is critical to maintaining continuity of care. Medication errors in prescribing, dispensing, or administration of a drug may cause harmful effect, and this is one of the most preventable causes of patient harm.

Marisa Webber, Nurse Medicines Management, from Hywel Dda UHB, said: ā€œBringing medications with you to the hospital will help clinicians clarify the medicines to be prescribed, the dose to be administered, and routes of administration. Not bringing medicines with you may cause a delay, which could mean you miss a dose of medication. This could hinder recovery and prolong a stay in the hospital.ā€
